Missouri Case Study: Building a Data-Driven Culture in a World of Value-Based Care (VBC)

As the Certified Community Behavioral Health Clinic (CCBHC) program expands, many states and stakeholders are identifying new ways to optimize CCBHCs in their state. Missouri was one of the original demonstration states that began this journey in 2015, with a SAMHSA planning grant, and then later expanded to implement CCBHCs statewide by 2023. Panelists will share their  perspectives from a provider and state lens from implementation to statewide success enhancing the quality of care and outcomes for their population, while also advocating for and leveraging CCBHC funding.  Find out how they’ve used data and technology to drive best practices, care coordination, population health and advocacy for funding opportunities across CCBHC and other statewide programs.

Through a focus on cross-functional partnerships within the organization and subagencies, as well as building community partnerships with resources outside of traditional mental health services, our TAY Case Management and Early Psychosis teams have been able to multiply access points for young adults in need of support, offer diverse and engaging programming options, and create a supportive, innovative, and collaborative working environment for staff. Establishing processes and a culture of regular case coordination has played an important role in creating these positive changes and resulted in higher levels of client retention and better health outcomes.
