Collaborating with Hospital Systems to Manage Complex Care Patients Presenters: Regional One Health Chief Integration Officer Susan Cooper, Manager Complex Care Megan William, and Alliance Healthcare Services CEO Laurie Powell LCSW One Health, developed by Regional One Health (ROH), is a complex care program designed to meet the needs of uninsured, medically and socially complex individuals. Alliance Healthcare Services (AHS) created a model to address barriers that resulted in complex individuals who repeatedly present in their crisis continuum, emergency departments, psychiatric inpatient hospitals and the criminal justice system. Many of the complex individuals were known to frequently engage with both institutions. To be successful in coordinating and collaborating, it was necessary to build an authentic relationship between ROH and AHS that allowed each partner to utilize their expertise to the fullest. In this workshop, you will learn about how these systems interface and gain insight into tools used to create a new ecosystem between healthcare, behavioral health, and social services. Most importantly, you will learn what true authentic collaboration looks like. |

You’ve Built a Law Enforcement – Behavioral Health Collaborative, Now What? Presenters: Bright Harbor Healthcare Services Director of Adult Clinical Services Kimberly Veith, and Director of Marketing and Development Meghan Corrigan Learn the history behind the first municipal Police/Social Work collaborative and how it grew to include several municipalities by fostering relationships with Police Chiefs, Town Councils, and The Prosecutor’s Office. Beyond the initial collaboration came the opportunity to build community wellness programs, Officer healthcare programs, and educational programs for both law enforcement personnel as well as the community. There has never been a better time to partner with our law enforcement colleagues, and our expertise as behavioral healthcare leaders can lend to building strong, healthy communities. |

The Value of Whole-Person Care: Using Analytics to Drive True Value-Based Care Presenters: I Am Boundless Vice President of Programs Jennifer Riha, Afia, Inc. Vice President of Analytics Strategy Melanie Elliott Not long ago, Boundless identified a greater need in the populations they serve for not just managing care among providers, but to provide whole person, integrated care to those individuals themselves, resulting in an application to provide physical health services and apply to become a Federally Qualified Healthcare Center. This presentation will focus on the road that Boundless traveled to get where they are now and how analytics helped them get there. In particular, the presentation will focus on: monitoring and reducing polypharmacy, clinical pathways to help individuals obtain and maintain healthy weight and BMI, calculating and monitoring various payer-specific requirements, calculating and monitoring the cost of care, integration of Health Information Exchange data to measure population health, identify risk among clients, and provide best practice care to treat those risk-based health issues, all while monitoring and managing exposure to COVID among residential and office clients and staff. These practices have provided critical insight to not only know the real-time state of affairs at Boundless, but to proactively plan and be able to act in the best interest of clients to ensure their health and wellness, which allows Boundless to prove to stakeholders and payers that their care models are true value-based models. |

Engaging Every Team Member, Every Day, in Improving Client Care Presenters: Sound Quality Improvement Specialist Brian Armbrust, Vice President & Chief Operations Officer Guy Delisi, Lean Coach Mike De Luca and President & CEO Patrick Evans Like any healthcare organization, Sound must be able to demonstrate that the people we serve are receiving value from the services we provide – to be relevant, our care for our clients must result in a measurable improvement in their outcomes. Sound turned to Lean (which we call Sound Thinking) to create a reliable and sustainable approach to improving clinical and operational results because of its dual emphasis on creating value for clients while engaging “every team member, every day” in improving their work. In this breakout session, we will share what we have tried in implementing Sound Thinking, what we expected to happen, and show how our learnings from what actually happened has shaped the evolution of our understanding and approach to this transformative work. You’ll learn how the role of leaders has been at the center of what we’ve achieved so far, and how deepening our leaders’ Sound Thinking leadership skills is critical for our future success. |

Adapt And Pivot - Applying Lessons Learned As We Move Forward Into This Next Decade Presenters: Netsmart COO Tom Herzog; Carl Clark, President and CEO, Mental Health Center of Denver; Susan Garnett, CEO, MHMR of Tarrant County; and Tracey Roxby, Netsmart VP, Client Alignment The notion has been that we move at the speed of thought, now the ask is for us to iterate at that same pace. The past 18 months have presented challenges, exposed areas for growth, and offered us the opportunity for continuous improvement. In this session we will discuss: ensuring we do not lose what is core to our teams in moments of uncertainty, utilizing resources and tools as a means to provide alignment, understanding what worked and did not work, building an essential framework and logistics as a foundation, and recognizing curiosity and anticipation as a strategy. We will review practices that can be applied to everyday situations that span from an urgent need to significant changes in a moment for a team, organization, or the communities we serve. |
