Presenters: Regional One Health Chief Integration Officer Susan Cooper, Manager Complex Care Megan William, and Alliance Healthcare Services CEO Laurie Powell LCSW

One Health, developed by Regional One Health (ROH), is a complex care program designed to meet the needs of uninsured, medically and socially complex individuals. Alliance Healthcare Services (AHS) created a model to address barriers that resulted in complex individuals who repeatedly present in their crisis continuum, emergency departments, psychiatric inpatient hospitals and the criminal justice system. Many of the complex individuals were known to frequently engage with both institutions. To be successful in coordinating and collaborating, it was necessary to build an authentic relationship between ROH and AHS that allowed each partner to utilize their expertise to the fullest. In this workshop, you will learn about how these systems interface and gain insight into tools used to create a new ecosystem between healthcare, behavioral health, and social services. Most importantly, you will learn what true authentic collaboration looks like.

Presenters: I Am Boundless Vice President of Programs Jennifer Riha, Afia, Inc. Vice President of Analytics Strategy Melanie Elliott

Not long ago, Boundless identified a greater need in the populations they serve for not just managing care among providers, but to provide whole person, integrated care to those individuals themselves, resulting in an application to provide physical health services and apply to become a Federally Qualified Healthcare Center. This presentation will focus on the road that Boundless traveled to get where they are now and how analytics helped them get there. In particular, the presentation will focus on: monitoring and reducing polypharmacy, clinical pathways to help individuals obtain and maintain healthy weight and BMI, calculating and monitoring various payer-specific requirements, calculating and monitoring the cost of care, integration of Health Information Exchange data to measure population health, identify risk among clients, and provide best practice care to treat those risk-based health issues, all while monitoring and managing exposure to COVID among residential and office clients and staff. These practices have provided critical insight to not only know the real-time state of affairs at Boundless, but to proactively plan and be able to act in the best interest of clients to ensure their health and wellness, which allows Boundless to prove to stakeholders and payers that their care models are true value-based models.

Therapy Connect – On-Demand Virtual Therapy Services
Therapy Connect lets you skip the commute, stay home, and limit your exposure to COVID-19 and other hazards! Long since a reality for physical healthcare, our innovative Therapy Connect portal allows clients to connect virtually with a master’s level clinician within less than 15 minutes. This on-demand therapy portal provides access to services when no onsite appointments are available due to staff shortages, for urgent, but not emergent, care and to address short-term clinical needs. Therapy connect affords both clients and clinicians the opportunity to stay at home thus extending a chicanes ability to work remotely without loss of productivity.

Moving away from Tradition into the Classroom
This presentation entails a partnership venture between South Community and a rural School District to bring Specialized Day Treatment into their educational setting, working with those youth who are too severe for traditional Day Treatment programs coupled with the goal of keeping the youth in his/her least restrictive school setting. This model supports specialized day treatment and full academic learning, different than traditional Day Treatment.
