Media Release
I certify that my signature (or checking the box) below grants Mental Health Corporations of America, Inc., dba mhca ("mhca") a world-wide, non-exclusive. royalty-free, perpetual. non-terminable license to use my stories, interviews. photographs. artwork, videos and audio (collectively, 'Works") along with my voice, identity or likeness used therein that I provide to mhca for use in its communications, marketing and PR efforts. I understand and confirm that any Work on any media created by me, mhca or a vendor can be distributed to mhca's internal and external audiences when and how they see fit, including its/their release, publication, exhibition, distribution, modification or reproduction to be used for news, webcasts, promotional purposes, telecasts, advertising, inclusion on websites or any other purpose by the mhca, its affiliates and representatives. I expressly release mhca, its officers and employees and each and all individuals involved from any liability connected with the taking, recording, digit izing. publication or distribution of such Works.
I hereby expressly waive any right I may have to inspect and/or approve the finished product derived from my Works, the copy that may be used in connection therewith, or the use to which it may be applied. I do hereby grant to mhca, and its employees and agents, a worldwide, perpetual, non-terminable, royalty-free license to use, display and distribute the Work in print and electronic form, publicly or privately. I expressly waive any rights, claims or interest I may have to control the use, display or distribution of my Works, or of my voice, identity or likeness therein in any and all media now known or hereafter devised, worldWide in perpetuity, as it pertains to my submissions of Works and/or event or program participation.
I understand there will be no financial or other remuneration for my contribution of Works, either for initial or subsequent transmission or playback. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release and understand that I am giving up certain rights by signing it. I am signing the release freely and without any inducement or assurance of any nature and I understand and acknowledge that it is binding on me and my heirs, assigns and personal representatives. Finally. I intend the release herein to be a complete and unconditional release of all liability to the greatest extent allowed by law.
